FAA Aviation Medical Examinations

Dr. Hardy is a Senior FAA Aviation Medical Examiner (AME) and provides examinations for First, Second and Third-class airmen. He has provided thousands of Aviation Medical Examinations during his 18 years as a civilian AME and previously as a U.S. Navy Flight Surgeon. He has extensive experience with the FAA guidelines and requirements for medically certifying pilots of all types, from the newest student pilot to the mid-career commercial pilot to the most seasoned airline pilot. Dedication to the health and safety of the aviation community is the focus of his practice.

Flight physicals are by appointment. We have offices in Salt Lake City and in Orem. We are in Salt Lake City on Tuesday and Thursday, and in Orem on Wednesday and Friday.

*** IMPORTANT: There are medical conditions and medications which require status reports from your treating provider in order for you to be medically certified. You will need to provide this at the time of your exam. When you schedule your appointment we will provide instructions regarding these reports. There will be a nominal additional fee due to the significant time required to manage these cases once the exam is complete.

*** Prior to your exam please go to medexpress.faa.gov. At this website you will need to create an account and then proceed with an Application for Medical Certification. When you complete the data entry and submit you will receive a confirmation number. You will need to bring the confirmation number with you to your exam.

*** The fee for an Aviation Medical Examination in our office is $150 for current patients and $175 for new patients. If you need an EKG (1st Class exam and over age 35) there is an additional charge of $50. (Additional fees are also added for exams requiring significant additional reports which must be submitted to the FAA for review.) Our practice does not have contracts with insurance companies, so please plan on paying with a credit or debit card, HSA card, or cash.
